Hitting Zero Hit Points

When a PC is reduced to zero (0) hit points, there are two possible outcomes:
  1. If the PC has some way to regain lost hp, whether through potions, spells, herbs, abilities, or traits, ze has to narrate dropping out of the action in the next turn to use the healing effect. This takes a whole turn, but the character is fine afterwards, as if ze didn't drop to zero hp.
  2. If the PC has no way to restore hir hp, ze falls Unconscious. This state may last for a few seconds or for hours.

Note that, no matter how much damage the final blow deals, it doesn't reduce the PC's hit points below zero.

Wound Saves

When a PC starts hir turn with 0 hit points, ze gains the Bloodied condition and must begin a Wound Saving Throw to see how bad the damage is.
A Wound Save is a series of Constitution checks the player makes once per turn, but the PC never adds hir Proficiency Bonus to the roll.

If the PC generates three successful checks first, the Wound Save is successful. The blow that felled the character was minor, knocking hir senseless for a few turns but without any further effect. Ze regains 1 hit point and awakens on hir next turn. If the PC generates three failures before three successes, the Wound Save fails. The blow that felled the character also caused a wound significant enough to put hir out of the fight: a badly bleeding wound, a concussion, a bruised nerve, a sprained joint, pulled muscle, or even a bone fracture. Magical healing will cure you quickly, although you may be too tired to be much good until you get a full night's sleep.

After the PC gains it, the Blooded condition persists until the next Short Rest.

Rolling 1 or 20

If a player rolls a natural 1 on a Wound Save, no matter what the result, it counts as two failures. If ze rolls a natural 20, the Wound Save ends in an immediate success.

Being Bloodied

Going down more than once in the same combat is a bad idea. While you're Bloodied, the DC for all your Wound Saves increases by 5. (i.e., DC 15 instead of DC 10) It's not cumulative at the moment if you go down repeatedly, but we may change that if it gets abused.
If you fail a Wound Save while you're Bloodied, the wound is worse than if you'd failed your first Wound Save. It may involve severed limbs, nerve damage, organ damage. It's the kind of thing that takes higher-level Cure spells to fix.
